ACCIDENT REPORTING FOR OSU EMPLOYEES OR STUDENTS WHO ARE EMPLOYEES

    To ensure proper reporting of work-related injuries and illnesses, The College of Biological Sciences has designated the College Safety Coordinator to maintain and record all work-related injury report forms and the electronic log known as the OSHALOG.

    In the event of a work-related injury or illness, the "Employee Accident Report" (EAR) must be filled out completely and immediately submitted to the Safety Coordinator in  Room 119 Biosciences.

    If medical treatment is needed, the employee should be sent to Employee Health Services (EHS) University Hospitals Clinic 2-A Lobby, Room 2018A on West 10th Avenue, taking the EAR with them.  If EHS is closed or emergency treatment is necessary, the employee should be sent to the OSU Emergency Department.  In the event that an employee receives medical treatment at another facility for a work related injury, an EAR still must be filled out completely and the employee must also be sent to EHS for an evaluation to assure that they are ready to return to work or to determine if any restrictions are needed.  Both EHS and The OSU Emergency Room have EAR forms available which simplify reporting.

    The Supervisor Follow-Up Report portion of the EAR also must be filled out immediately after the accident.  The supervisor or person to whom the injured employee reports directly is required to fill out the Supervisor portion and return it to The Safety Coordinator within 72 hours.  This is designed to promote understanding of the accident's cause and future avoidance.

    Copies of the EAR  need to be forwarded to the Safety  Coordinator  immediately as the accident must be recorded into the OSHALOG within six working days of its occurrence.   EAR  Forms are available in Word or in PDF at the top of this page.

ACCIDENT REPORTING FOR STUDENTS

   Students who are injured in a classroom should seek appropriate medical care.  In minor injuries, first aid can be administered by the instructor or supervisor.  In the event of injury requiring medical attention, the student should be directed or escorted to the Wilce Student Health Center, 1875 Millikin Road, or, if the injury is severe, directly to the OSU Hospitals Emergency Room.   An incident report in the form of an Employee Accident Report Form detailing the injury, the circumstances, and ways of future avoidance should be forwarded to The College Safety Coordinator, Room 119 BioSci within 72 hours of the accident.
